WE MUST HAVE THE HOME ADDRESS OF THE PRESIDENT, SECRETARY AND TREASURER. (WE DO NOT SHARE THIS INFORMATION WITH NON-PTA ENTITIES.) THE INFORMATION PROVIDED IS WHAT WE USE TO COMMUNICATE WITH EACH LOCAL UNIT. THANK YOU.
PLEASE COMPLETE all required information (as soon as officers are elected). You will not receive membership cards and the Florida PTA Kit of Materials if this information is not updated each year. Information MUST be completed for President, Treasurer and Secretary, INCLUDING HOME ADDRESSES.
Membership Cards and the Florida PTA Kit of Materials will be sent to each local unit upon receipt of this completed form. (ALL MAILINGS WILL BE SENT TO THE PTA/PTSA PRESIDENT’S HOME ADDRESS YOU PROVIDE.)
AFTER COMPLETION OF YOUR FORM: Check your email for the email confirmation of the form submission. After confirmation is received, the State Office will begin the process to send out the shipment of the membership cards with your Kit of Material Instructions. (Remember, these will go to the PTA/PTSA President’s mailing address that you provided.)
