If you are registering officers for the 2019-2020 fiscal year, you will register your officers using your PTA’s Hub in MemberHub.  The 2018-2019 President should have log-in information for your local unit’s Hub.  If that information is not available, please email

**Please note**:  If you have already submitted your officers for the year but are CHANGING any of the submitted officer names, please submit a Change of Officer Form.

PLEASE COMPLETE all required information (as soon as officers are elected). You will not receive membership cards and the Florida PTA Kit of Materials if this information is not updated each year. Information MUST be completed for President, Treasurer and Secretary, INCLUDING HOME ADDRESSES.

The Florida PTA Kit of Materials will be sent to each local unit upon receipt of this completed form. (ALL MAILINGS WILL BE SENT TO THE PTA/PTSA PRESIDENT’S HOME ADDRESS YOU PROVIDE.)

AFTER COMPLETION OF YOUR FORM: Check your email for the email confirmation of the form submission. After confirmation is received, the State Office will begin the process to send out your Kit of Material Instructions. (Remember, these will go to the PTA/PTSA President’s home mailing address that you provided.)
