2 – Form DR5 (Florida Sales Tax Exemption Certificate);
3 – Standard 501(c)(3) letter for donation requests;

Officer Registration

If you are needing to update your officers for 2019/2020, please use the New/Returning Officers Registration Form – located at the top right/hand corner of the website.  If you need to change your officers, after initial submission, please email:  membership.data@floridapta.org, for specific instructions on how to update officers.

Membership Cards

Printed membership cards will not be provided this year.  All membership cards are sent electronically through MemberHub.
