FOR PTA/PTSAs THAT HAVE ALREADY RECEIVED THEIR MEMBERSHIP CARDS
PLEASE COMPLETE AS MUCH INFORMATION AS POSSIBLE and send to the State Office (as soon as officers are elected). YOUR OFFICER INFORMATION MUST ALWAYS BE CURRENT AS REQUIRED BY THE IRS GUIDELINES.
AFTER COMPLETION OF YOUR FORM: Check your email for the email confirmation of the form submission.
  • Select the year you are serving as officer of PTA/PTSA
  • As listed in your bylaws
  • (i.e. Orange, Pinellas, Seminole, etc.)
  • ALL OFFICER INFORMATION MUST BE COMPLETE TO INCLUDE THE OFFICER'S NAME, HOME ADDRESS, PHONE NUMBER AND EMAIL ADDRESS
